Course Implementation Questionnaire

Please provide the following information:

  • Name of contact person
  • Affiliation
  • Mailing address
  • Phone; fax; and e-mail

Please provide brief answers to the following questions:

  1. Who is the project director (the person who will work with Bard College to develop community interest, select the host site and course director, and design the fundraising plan)? Describe this person’s background and credentials. Provide contact information if different from above.
  2. Has a host site been identified? If so, describe the location, classroom and childcare facilities, population served, and ability to recruit students, provide administrative support, and raise funds specifically for the Clemente Course. How does the course fit the mission of the host organization? Attach a mission statement, organizational newsletter, and program announcements.
  3. Who is the target population? Is there anything distinctive about this population and do they have any special needs or goals?
  4. Have you identified a candidate for course director? If so, provide a description of this person’s background and credentials.
  5. What are the colleges and universities in your community from which experienced faculty can be drawn?
  6. What funding possibilities exist in your community to support and sustain a Clemente Course?
  7. As you see it, what is the community need and how will a Clemente Course fit with existing services and opportunities?
